This Licensed Vocational Nurse (LVN) position involves providing direct patient care under the supervision of a Registered Nurse in a family medicine outpatient clinic and urgent care center setting. The LVN will perform a variety of clinical tasks, including patient assessments, medication administration, and assisting with procedures, while collaborating with the healthcare team. Floating to other family medicine locations is required.

Responsibilities:
- Rooming patients and obtaining patient history, allergies, and vital signs.
- Administering medications (oral, subcutaneous, IM, SQ).
- Performing phlebotomy, EKGs, and other diagnostic tests (e.g., urine collection, throat swabs).
- Assisting with wound care and simple dressing changes.
- Performing injections and administering ear and eye drops.
- Managing patient pain and providing pain assessment.
- Performing telephone triage and patient follow-up calls.
- Managing patient medication reconciliation and refill requests.
- Assisting with NG tube/G tube insertion and maintenance.
- Administering oxygen and performing other respiratory treatments.
- Assisting with patient education and discharge instructions.
- Maintaining accurate patient records and documentation.
- Collaborating with the healthcare team to provide comprehensive patient care.
- Working independently and efficiently in a fast-paced environment.
- Adhering to all safety and infection control protocols.
Required Skills & Certifications:
- Current LPN/LVN license in the state of employment.
- BLS certification.
- Proficiency in performing procedures listed in the responsibilities section.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Experience in outpatient clinic/urgent care settings preferred.
- Ambulatory and pediatric experience preferred.
Preferred Skills & Certifications:
- Experience in emergency or critical care environments.
- Experience with vents, trachs, and PICC lines.
- Two years of experience in direct patient care.
Special Considerations:
- Floating to all other Family Medicine locations is required.
